Kinds of reportable tasks
Report types
You can see the report types available to you by clicking the blue Report Something button above your site inbox. This is used to report events such as incidents. Additional report types can be enabled beyond the default options available to you, as shown in the table below.

| A problem that can be corrected or prevented (PCA) | Manufacturing | This template records preventive and corrective actions in accordance with ISO 9001. Preventive actions address potential nonconformities, while corrective actions prevent recurrence of existing nonconformities. |
| A suggestion | Any | Log a suggestion. Can be used for employees to be able to suggest anything, e.g. improvements to the workplace that are not safety related. |
| Alleged food allergy | Food | Log an alleged food allergy. |
| Alleged food poisoning | Food | Log an alleged food poisoning. |
| Alleged foreign object | Food | Log an alleged foreign object. |
| Contractor on site | Any | Used to record the arrival and departure of a contractor. This report also contains key steps that should be carried out before a contractor is allowed to work on site. |
| Corrective action | Any | Use this template to manually create an action in the system, such as adding sub tasks to an incident report or corrective actions for an asset with defects. |
| Dynamic risk assessment | Any | This template give reporters the opportunity to complete a dynamic risk assessment and look for hazards that are specific to a site or working area. |
| Employee intervention | Any | Use this template to record actions required for an employee, typically following a near miss or accident caused by unsafe behaviour or an omission. It can also be used for issues such as PPE non-compliance or required retraining. |
| Enforcement officer visit | Any | Log a visit from an enforcement agency such as the Fire Service, HSE, Environmental Health etc. Log the visit and add actions as sub tasks to ensure that you complete any required further actions. |
| Fire evacuation or drill | Any | Logging of ad-hoc fire evacuations or drills |
| First aid administered | Any | This template is used to log first aid given to members of the public where an accident hasn't occurred on your premises. Typically used by clients with high numbers of visitors on site |
| Food delivery | Food | This template is for logging food deliveries to check the acceptability of the delivery's condition and food temperatures |
| Food manufacturing contamination | Food Manufacturing | This template is for logging food manufacturing contaminations, with an investigation and product recall section. |
| Incident (Injury / Near miss) | Any | Our largest template on offer. This gives the reporter the opportunity to log an incident, such as an injury or near miss. After logging, detailed injury (if applicable) and investigation sections will generate to allow managers to collect the necessary information. |
| Maintenance request | Any | Logging of issues that require maintenance. |
| Pest activity | Food | This template offers the reporter the opportunity to report pest activity on site. |
| Plant / Machine incident | Merchants Manufacturing | Use this template to report damage to plant or machinery. |
| Road traffic collision | Any | Log vehicular incidents on public highways with this template. For on-site incidents, use a different template. |
| Safety improvement opportunity | Any | This template offers the reporter the opportunity to raise improvements to safety, asking questions such as "Please describe the opportunity" and "Who or what will be impacted positively by this improvement?" |
| Safety observation | Any | Use this template to report potential workplace hazards, such as damaged equipment or PPE non-compliance. Remember, a Safety Observation identifies something that could cause harm, while a Near miss (in the Incident form) records an incident that occurred but caused no harm. |
| Security incident | Any | Log security incidents including theft, damage and trespass. |
| Something positive | Any | This template gives the reporter the opportunity to log something positive, e.g. observed good working practices. |
| Spillage | Merchants Manufacturing | Designed for recording spillages of substances. This could be particularly beneficial for clients who are ISO 14001 accredited and require detailed spillage documentation. |
| Task observation | Any | This template is used to observe tasks as part of a spot check to ensure correct procedures are being observed. |
| Visitor on site | Any | This template is similar to the Contractor on site, but for logging general visitors on site. |
